I, for the player and I, hereby release the Host Facilities, the Alabama Junior Golf Organization , its officers, directors, and employees from any and all liability for any event or consequence whatsoever in any way arising out of or relating to member's participation in a Alabama Junior Golf Organization event or activity with the sole and singular exception of liability arising out of bad faith or willful misconduct. In case of emergency during a Alabama Junior Golf Organization event, I authorize a qualified medical physician to take necessary measures in the treatment of this AJGO participant. It is understood that neither the Host Facilities nor the Alabama Junior Golf Organization is responsible for any lost or stolen items. Signatures verify both the parent/guardian have read and understood the Organization Rules as described in this application.
I, for the player and me, hereby have read and understand the AJGO Withdrawal Process, its rules, regulations, and all fees that may apply. I understand that NO refunds will be granted with less than 7-days prior to the tournament, unless certain criteria are met. I also understand that there will be a $10 processing fee for all refunds granted, with the exception of fees being rolled to another tournament, in which no fees apply. Signatures verify both the parent/guardian have read and understood the AJGO rules as described.
